Patient Rights and Responsibilities

 
A person who receives health services in HSHS Good Shepherd Hospital, within the framework of Catholic medical moral teaching and other regulatory requirements, certain rights and responsibilities as a patient. In the case of patients under the age of 18, or patients unable to make decision on their own, the person who is legally responsible for the patient must see that these rights and responsibilities are met.
 
You have the right to:
  • Be kept informed in a language or manner that you, or your representative, can understand
  • Access to language interpretation services, and assistance with visual, hearing, speech or learning difficulties
  • Not be discriminated against due to race, color or national origin
  • Know the name of the physician responsible for coordinating your care, as well as names and roles of others involved in your care
  • Receive care in a safe setting free from all forms of abuse, neglect or harassment in a dignified and respectful manner
  • Be free from restraints or seclusion in any form imposed as coercion, convenience, or retaliation
  • Care based on sound nursing and medical practice
  • Participate in the development and implementation of your plan of care
  • Pain management
  • Be provided with information about your diagnosis, proposed treatments, procedures, outcomes, including unexpected outcomes in the event they may occur,  and health status so you can make informed health care decisions
  • Request or refuse any treatment in accordance with law and regulation, this right is not a mechanism to demand the provision of treatment or services that are determined to be medically unnecessary or inappropriate
  • Be informed of the nature of your condition,
  • Be informed of any proposed treatment or procedure, risks, benefits and prognosis, and any continuing health care requirements after your discharge in terms you can understand.
  • Be informed of medical alternatives for care or treatment.
  • Refuse treatment, except as otherwise provided by law, and to be informed of the consequences of your refusal.
  • Have family involved in your care, treatment and service decisions to the extent permitted by you or your designated surrogate decision-maker in accordance with law and regulation
  • Make an advance directive and appoint someone to make health care decisions for you if you are unable to do so.  If you do not have an advance directive, we can provide you with more information and help you complete one
  • Have hospital staff and practitioners who provide care in the hospital comply with your advance directive in accordance with State Law and hospital policy.  As a Catholic hospital your choices must be consistent with the hospitals philosophy and values for those services to be provided.
  • Have a family member or person of your choice, and your own doctor notified promptly of your admission to the hospital
  • Personal privacy and confidentiality
  • Privacy and confidentiality of personal health information except as otherwise permitted by law
  • Access information in your medical record within a reasonable time frame
  • Access, request amendment to, and obtain information on disclosures of your health information in accordance with law and regulation
  • Examine and receive a reasonable explanation of your total bill
  • Consent or reject participation in a research program, investigational study and clinical trials as well as the right to receive an explanation of the nature and possible consequences of such research
  • Respect for your cultural and personal values, beliefs and preferences
  • Religious and other spiritual services
  • Give, or withhold informed consent to produce, or use recordings, films or other images for purposes other than your care
  • Access to protective and advocacy services in cases of abuse or neglect
  • Consent to and receive visitors of your choice, including but not limited to a spouse, domestic partner, another family member or friend.  You have the right to withdraw or deny consent at any time.  A support person of your choice may accompany you during your stay unless that person’s presence infringes on others’ rights, safety or is medically or therapeutically contraindicated. Support person must be able to function independently and care for themselves. Visitors are welcome 24 hours a day in most areas with the exception of some special care areas
  • You have the right to file a complaint or grievance without being subject to coercion, discrimination, reprisal or unreasonable interruption of care
If your concerns are not adequately addressed by nursing staff, please ask to speak with the department manager.  If those directly involved with your care cannot resolve your concern, call the patient experience manager at ext. 8592 or the director of quality at ext. 8204.

  • Provide complete and accurate information about your health and medical history
  • Ask questions and acknowledge when you do not understand information or instruction given to you and actively participate in your treatment plan
  • Observe polices, rules and regulations in place to support quality care and a safe environment for all
  • Treat all hospital staff, other patients and visitors with consideration and respect
  • Provide complete and accurate information about health insurance coverage and meet financial obligations
